  databases/couchdb3: use packaged rebar

Use the packaged rebar for the upstream release build instead of the bundled escript, whose embedded BEAM files fail to load under the Erlang runtime used by the port.

Also fix staged install paths for runtime directories and the Fauxton UI copy.
